Vital Records (Birth, Death, Marriage)

Note that I cannot notarize or authenticate copies of vital records documents. This must be done by the County Clerk.

  • Certificate of Identity/Sworn Statement (For requesting a copy of a birth or death certificate)
  • Affidavit of Inability to Appear (For Marriage License applications for the home bound, hospitalized or incarcerated)
